Responsibilities:- Examine patients, take medical histories, maintain patient records, identify health risk factors, prescribe medications, make referrals, and treat health conditions
- Provide direct patient care within a skilled nursing facility setting, working in conjunction with APRNs who manage acute visits and assist with chronic visits
- Focus on patient wellness and disease prevention, prevention of hospital readmissions, antibiotic stewardship, advance care planning, collaboration with families and rehab service providers
- Provide health education services and counsel patients on healthy lifestyle choices
- Risk Management: understand the importance of documenting and addressing concerns such as risk of falls, malnutrition, polypharmacy, and pressure injury
- Understand and comply with the Environment of Care/Infection Control Responsibilities
- Is knowledgeable and complies with the States Institutional Activities and Expectation
