Nurse Case Manager

We are seeking a highly skilled Registered Nurse Case Manager to join our team at Health Care for the Homeless. As a key member of our healthcare team, you will provide comprehensive care to individuals experiencing homelessness, focusing on their medical and social needs.
Key Responsibilities:- Provide nurse case management services to clients, including medical care coordination, linkage to health-related resources, and monitoring referral outcomes.
- Deliver health education and counseling to clients, drawing upon their strengths, resiliency, and motivation.
- Perform clinical tasks, including nursing assessments, nursing treatments, medication administration, and adherence support.
- Conduct home visits, community visits, and escorts independently and in accordance with clients' plans of care.
- Demonstrate positive regard and respect for clients through speech and actions, utilizing a client-centered, non-judgmental, trauma-informed approach.
- Involve clients in the development and implementation of integrated treatment plans using SMART goals.
- Complete documentation within clients' electronic health records in a manner that is easy to understand and in accordance with established formats and required timeframes.
- Bachelor's Degree from an approved School of Nursing.
- Licensed in Maryland as a Registered Nurse.
- Personal vehicle and valid Maryland driver's license.
- Two years of case management experience preferred.
- Two years of clinical nursing experience.
- Experience working with individuals who have behavioral health disorders preferred.
