Job Title: Licensed Practical Nurse - Bronx Transitional Housing

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at the Center for Urban Community Services (CUCS) in Bronx, New York. As an LPN, you will play a vital role in providing supportive services to individuals experiencing homelessness and mental illness.

Job Summary:

The LPN will work collaboratively with our multidisciplinary team to provide comprehensive healthcare services, including medication management, wound care, and health education. You will also assist with escorting patients to healthcare appointments, compiling patient health information, and maintaining medical office equipment.

Responsibilities:
  • Manage and lead the Assistance with Self-Administration of Medication (AWSAM) program
  • Provide nursing care at the order of a Janian provider, including administering injections, assisting with wound care, and supporting client health behaviors
  • Support the primary care and psychiatry programs at the site, working collaboratively with site social work staff
  • Participate in staff meetings and other case conferencing meetings as scheduled
Requirements:
  • LPN license required
  • Experience working with people living with mental illness and/or with homeless or justice-involved people preferred
  • Excellent verbal and written communication skills and computer literacy required
  • Close attention to detail is essential
  • Bilingual English/Spanish preferred, but not required
