Key Responsibilities- Provide comprehensive nursing services to individuals with disabilities, including assessment, planning, intervention, and evaluation.
- Develop and implement individualized care plans in collaboration with the healthcare team and individuals served.
- Conduct regular health assessments and provide health education to individuals and their families.
- Collaborate with community resources and healthcare providers to ensure seamless care coordination.
- Participate in interdisciplinary team meetings to discuss individual care plans and share best practices.
- Document all interactions and care provided in a timely and accurate manner.
- Maintain accurate and up-to-date medical records and reports.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Bachelor's Degree in Nursing or equivalent experience.
- Valid RN license and current CPR certification.
- Minimum of one year of experience providing nursing services to individuals with disabilities.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and critical thinking skills.
