Role Summary

The Nurse Practitioner (NP) is a highly trained registered nurse with advanced education and clinical expertise, responsible for delivering comprehensive hospice services in accordance with state regulations and under the guidance of a collaborating physician. The NP plays a crucial role in hospice care by assessing health histories, promoting health, diagnosing medical conditions, and managing health issues through the development of care plans, prescribing necessary medications or treatments, and collaborating with patients, families, and other healthcare professionals.

Key Responsibilities
  • Conduct patient visits, perform thorough physical assessments, and evaluate hospice eligibility in collaboration with the interdisciplinary team.
  • Utilize community resources to effectively plan and deliver services tailored to the needs of individuals and families.
Qualifications
  • Graduation from an accredited School of Nursing.
  • Completion of an accredited Nurse Practitioner Program.
  • Current nursing licensure in California and CPR certification.
  • Master's degree with at least one (1) year of Nurse Practitioner experience and one (1) year in hospice or palliative care.
  • Accreditation from a recognized certifying body for advanced practice nursing as mandated by state regulations.
  • Certification in Hospice and Palliative Nursing (CHPN) is preferred.
  • Strong observational, verbal, and written communication skills, along with problem-solving and mathematical abilities.
