At Acenda Integrated Health, we are seeking a dedicated and compassionate Case Manager to join our Integrated Case Management Services team in Gloucester, New Jersey.

As a Case Manager, you will play a vital role in supporting individuals and families in their journey towards health and happiness. You will engage with consumers at the point of hospitalization, participate in discharge planning, and provide post-discharge follow-up to support community integration.

To be successful in this role, you will require a minimum of a Bachelor's Degree in Human Services or Psychology, and at least 2 years of experience in mental health services. You must also possess professionalism, a commitment to excellence in care, and a valid driver's license with an acceptable driving record.
