Nurse Practitioner/Physician Assistant

Job SummaryWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and developing personalized care plans to improve their quality of life.
Responsibilities- Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up care.
- Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
- Master's degree required or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.
- Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
