Job Overview:

About the Role:
This is an evening shift position, working 3:00 p.m. to 3:00 a.m., with rotating weekends and holidays. You will work closely with our interdisciplinary team to coordinate patient care, develop individualized treatment plans, and implement evidence-based practices.

Responsibilities:
• Provide direct and indirect patient care services, including vital sign collection, wound care, and medication administration.
• Collaborate with healthcare providers to ensure seamless transitions of care.
• Develop and maintain accurate, up-to-date records of patient interactions, medical history, and treatment plans.
• Communicate effectively with patients, families, and staff members to promote a culture of transparency and respect.
• Participate in quality improvement initiatives to enhance patient satisfaction and outcomes.

Requirements:
• Currently licensed to practice as a licensed practical nurse by state of employment or holds a multistate LPN license through the interstate Nurse Licensure Compact or eligible for licensure in state of employment.
• One (1) year of LPN experience preferred.
• Basic Life Support (BLS) certification required prior to completion of orientation.

Salary Estimate:
$64,500 - $77,400 per annum, based on relevant experience.
