Key Responsibilities
  • Evaluate patients for hospice eligibility and develop comprehensive care plans
  • Coordinate patient services, including medical, emotional, and spiritual support
  • Document patient assessments and care plans accurately and thoroughly
  • Work collaboratively with interdisciplinary teams to ensure high-quality patient care
  • Participate in on-call rotations and attend regular staff meetings
Requirements
  • Current RN licensure in the state of branch location
  • One year of clinical nursing experience
  • Strong communication, organization, and time management skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
  • CPR certification and minimum of twelve hours continuing education annually
