Nurse Care Specialist

At Matrix Medical Network, we're dedicated to delivering high-quality clinical services to at-risk health plan members in the comfort of their own homes. As a Nurse Practitioner - PRN/Part Time - Home Risk Assessments, you'll play a vital role in helping us achieve our mission.
Key Responsibilities- Conduct comprehensive health assessments, medical history reviews, and diagnosis/treatment planning for adult and geriatric patients
- Maintain accurate and detailed documentation of patient interactions and care plans
- Collaborate with physicians, case managers, social workers, and family members to provide patient-centered care
- Develop and implement effective health education plans to empower patients and their caregivers
- Utilize Matrix's comprehensive health assessment tools to identify care gaps and opportunities for improvement
- Master's Degree in Nursing or commensurate experience and licensure
- Current RN and NP licensure with prescription authority in the state of practice
- Board certification in a Matrix-approved specialty
- Minimum 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health record software
- Excellent verbal and written communication skills
As a PRN/Part Time Nurse Practitioner, you'll have the flexibility to choose your own schedule and work at your own pace. You'll have the opportunity to work with a diverse range of patients and healthcare professionals, and will be supported by Matrix's comprehensive training and resources.
Compensation and BenefitsWe offer a competitive hourly rate of $90.00 or $ (based on county) per completed assessment. You'll also have access to our mileage reimbursement plan and 401(k) benefits.
