Job Summary:The Rapid Engagement Delivery Specialist will provide support and guidance to engage and connect high-risk/high-need individuals to services as rapidly as possible. This includes assessing eligibility for financial assistance, facilitating enrollment, and working with the RED team process.
Key Responsibilities:- Assess eligibility for DHS temporary assistance and assist clients with obtaining assistance
- Gauge client level of need and make referrals to appropriate community providers
- Work with DSS financial assistance and emergency housing unit
- Provide care management services to clients, employing person-centered, motivational, and other evidence-based practices
- Initiate and follow up on referrals, paperwork, and telephone contacts needed for enrollees' immediate access to behavioral health and social services
- Document, enter, and maintain records of enrollments, dispositions, team meetings, trainings, etc.
- Conduct routine outreach to homeless shelters, client homes, clinics, and other appropriate venues
- Attend community meetings, staff meetings, and training related to job duties
- Bachelor's degree in mental health, social work, or related human services field or equivalent education and work experience
- Experience with a Windows-based operating system (Word processing, spreadsheet, and database management software)
