Part-Time Nurse Practitioner, In-Home Visits Specialist

We are seeking a skilled Part-Time Nurse Practitioner, In-Home Visits to join our Home-based Medical Care team in Brewton, AL. As a member of our team, you will play a vital role in bringing high-end medical service and compassionate care to complex, chronic patients in the comfort of their own homes.
Key Responsibilities- Manage chronic illnesses in a complex elderly population with multimorbid conditions, focusing on wellness, avoiding unnecessary hospitalizations and ER visits, and providing patient education about disease processes.
- Document timely, with an added focus on closing HEDIS quality gap measures and recapturing HCCs.
- Engage new patients enrolling in the program with a comprehensive geriatric-focused initial assessment, including functional evaluation, social and safety assessments, cognitive screening, and mental health assessments.
- Triage, diagnose, and treat patients on an urgent basis in their homes, utilizing an array of equipment and medications.
- Order and interpret appropriate tests, including x-rays, labs, and EKGs, to diagnose, prescribe, and treat within the provider's scope of practice and consistent with the standards of care.
- Start treatment in the home at the point of care and collaborate with patients' PCPs and specialists as needed.
- Bachelor of Science in Nursing (BSN), in addition to an NP-focused graduate degree in a Master's or Doctoral nursing program.
- Graduate from an accredited NP Program and have Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
- Active and unrestricted State Nurse Practitioner license for the State of Alabama.
- Current state RN license.
- Valid DEA license or the ability to obtain one prior to employment.
- Access to reliable transportation, with compliance with Optum's Motor Vehicle Safety policy.
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, palliative care, emergency/urgent care, or similar settings.
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
- Knowledge of CPT, ICD-10, and HCPC codes.
- Knowledge of clinical standards of care.
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
