Nursing Case Manager

San Gorgonio Memorial Hospital seeks a highly skilled Nursing Case Manager to join our team. The ideal candidate will have excellent communication skills and the ability to work collaboratively with staff, physicians, and payor representatives to ensure effective clinical resource utilization and documentation.
In this role, you will provide utilization education, identify inefficiencies in resource utilization, and act as a resource to staff and physicians. You will be required to consistently demonstrate effective communication skills and form collaborative relationships with patients, families, and healthcare teams.
Key Responsibilities:
- Manage clinical resource utilization and documentation effecting reimbursement.
- Provide utilization education and identification of inefficiencies in resource utilization.
- Act as a resource to staff and physicians for documentation purposes.
Requirements:
This position requires an ADN or BSN in Nursing (BSN preferred for new hires after December 2017), current CA RN License, and at least 3 years of minimum clinical nursing experience. Preferably, you should have 1-2 years of experience in Utilization Review (U.R.) or Case Management (C.M.). Knowledge and experience with Medi-Cal, InterQual (CERME) or other national standards are also desired. Additionally, you should possess data entry skills, basic computer skills, and excellent communication skills.

The estimated pay is based on qualifications, including but not limited to experience, education, certifications, and licensure where applicable.
