As a Home Healthcare Physical Therapist Assistant, you will provide physical therapy treatments to clients under the supervision of a licensed Physical Therapist, following a comprehensive Plan of Care. Your role involves conducting in-home visits, typically lasting 1-2 hours per visit, to ensure optimal care and support for our clients.

Key Responsibilities:

  • Deliver physical therapy services and education/training to clients in their homes, adhering to the physician's Plan of Care and the specific functions identified in the therapy care plan.
  • Complete visits in a timely manner and in accordance with Agency policy.
  • Document all services provided, observations, client progress, and recommendations on designated forms and submit within seven (7) days or per Agency policy.
  • Report changes/concerns or schedule changes to the therapist and/or nursing supervisor on the day of observation.
  • Participate in interdisciplinary activities per Agency guidelines.
  • Attend client care conferences as needed to provide information to other team members.
  • Promote referrals to other disciplines as appropriate.
  • Attend staff meetings and continuing education programs.
  • Contribute to Agency quality improvement activities pertinent to the role of Physical Therapy Assistant.
  • Promote personal safety and a safe environment for clients and coworkers.
  • Demonstrate knowledge of safety and infection control practices by compliance with Agency policies and procedures.
  • Recognize and report potentially unsafe situations.
  • Ensure safe practice in the use of equipment.
  • Assess the environment and initiate actions to prevent accidents and promote safety. Maintain confidentiality in all aspects of the job.
  • Do not discuss client or staff identities, conditions, or circumstances in public or with others who are not professionally involved with the situation/person.
  • Protect written confidential documents in a manner that prevents unauthorized access.
  • Adhere to all Agency policies about client privacy and security of information.
  • Perform job duties in compliance with Agency policies and procedures, Medicare, CHAP, and/or Joint Commission standards, and professional community standards.
  • Accept responsibility for personal and professional development. Identify learning/developmental needs.
  • Perform additional duties and responsibilities as deemed necessary.
