About the Role:

The RN Care Coordinator is a vital member of our healthcare team, responsible for assessing, planning, and facilitating care transitions for patients. This role requires strong communication and collaboration skills, as well as the ability to navigate complex healthcare systems.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Coordinate with healthcare providers, insurers, and post-acute service providers to ensure seamless care transitions
  • Advocate for patients and families, identifying and addressing cultural, language, and socioeconomic barriers to care
  • Protect patient confidentiality and maintain high levels of patient satisfaction

Requirements:

  • Minimum 3 years nursing experience in an acute hospital setting
  • Prior care coordination experience in a clinical or insurance setting
  • Current CA Registered Nurse (R.N.) license
  • Excellent customer service and presentation skills
  • Strong interpersonal and written communication skills

Preferred Qualifications:

  • CM certification
  • BSN degree or experience equivalent
  • Bilingual skills in other languages
