Job Summary

We are seeking a skilled Nurse (Care Coordinator) to join our team at the Indian Health Service. As a key member of our healthcare team, you will be responsible for coordinating patient care and treatments, improving patient outcomes, and promoting self-management of chronic disease.

Responsibilities
  • Organize patient care and treatments by incorporating all members of the care team.
  • Improve patient care outcomes by assessing patient needs and developing effective care plans.
  • Promote self-management of chronic disease by helping patients identify barriers to making healthful choices and providing coaching and guidance on positive health behaviors.
  • Assess, plan, implement, and evaluate the care of patients with medical, surgical, pediatric, newborn, obstetric, ambulatory, and emergency conditions in the primary care clinic.
  • Recognize adverse signs and symptoms and react promptly in emergency situations.
  • Provide chart review to identify patients at risk for increased morbidity and mortality and recommend interventions.
  • Provide patient and family education to assist in understanding and accepting the implications of care, diagnosis, and treatment.
Requirements
  • A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required.
  • Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
  • Pass the National Council Licensure Examination and possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
  • At least one full year of professional nursing experience (equivalent to the GS-9 grade level) is required.
