With a focus on providing high-quality, patient-centered care, you'll work closely with our multidisciplinary team to develop and implement individualized plans of care. Your expertise in IV therapy and infusion nursing will be invaluable in ensuring the best possible outcomes for our patients.

Responsibilities:

  • Provide professional nursing care to patients in their homes, adhering to policies and procedures addressing the scope of services outlined in infusion nursing services, pharmacy services, and state laws and regulations.
  • Collaborate with pharmacy staff, physicians, and patients to develop and monitor goals of patients' therapies.
  • Assess patients and establish and implement individualized plans of care.
  • Manage vascular access devices, including insertion of peripheral infusion devices or peripherally inserted catheters (if certified).
  • Educate patients and caregivers on infusion therapy and related care.
  • Evaluate progress toward goals and make recommendations to physicians for changes in the plan of care based on patients' needs.
  • Coordinate nursing care with home health and other community agencies as indicated.

Qualifications:

  • Graduate of an accredited school of nursing; Bachelor of Science in Nursing preferred.
  • Valid nursing license in state(s) of practice.
  • Minimum of one (1) year of experience as a Registered Nurse.
  • Previous infusion and home health care experience preferred.
  • Extensive experience in IV therapy preferred.
  • Current CPR certification.
  • Flexible schedule with the availability to work a variety of shift models.
  • Ability to work effectively within a multidisciplinary team.
  • Effective verbal and written communication skills.
  • Valid driver's license and auto insurance.
  • Must meet company standards for driving record as well as driver age requirement.

Salary Range: $55.00 - $58.00 per hour
