In this role, you'll deliver care to patients with end-stage renal failure or chronic kidney disease in an intimate outpatient setting. As a Dialysis Nurse Care Specialist, you'll build long-term relationships with patients and their families, providing empathetic and compassionate care.

About the Job

This position involves delivering care to patients who require unique treatment plans due to multiple co-morbidities. You'll work with a team that supports and relies on each other in a positive environment. Your schedule will be flexible, requiring mornings, evenings, weekends, and holidays.

Key Responsibilities

  • Become part of a community-first organization with a goal to embed Diversity & Belonging into everything we do
  • Work with patients who are often dealing with multiple co-morbidities that require unique treatment plans and a broad range of nursing skills and knowledge
  • Oversight of patient groups, supported by PCTs, for conducting observations, measuring stats, and machine setup
  • Build meaningful relationships with patients and their family members

Requirements

  • Associates Degree in Nursing (A.D.N.) from an accredited school of nursing required; Bachelor of Science in Nursing (B.S.N.); three-year diploma from an accredited diploma program may be substituted for nursing degree
  • Previous experience in ICU, Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) is beneficial but not necessary
