Job SummaryWe are seeking a compassionate and skilled Hospice Licensed Practical Nurse (LPN) to join our team. As a Hospice LPN, you will play a vital role in enabling patients to spend quality time with their loved ones and engaging in activities they enjoy. You will provide direct, skilled nursing services in accordance with the Plan of Care under the direction of the RN Case Manager.
Key Responsibilities- Provide direct, skilled nursing services to patients in their homes, focusing on comfort, peace, and dignity.
- Build trusting, compassionate relationships with patients and their families, empowering them to make informed decisions about their care.
- Collaborate with the care team to develop and implement individualized care plans that meet the unique needs of each patient.
- Participate in ongoing education and training to stay up-to-date on best practices in hospice care.
- Current LPN licensure in the state of employment.
- Experience in hospice care or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
