Job Description

**About the Role**

We are seeking a highly skilled and compassionate Occupational Therapist to join our team at Elara Caring. As an Occupational Therapist, you will play a vital role in delivering high-quality care to our patients in the comfort of their own homes.

Key Responsibilities:

  • Evaluate patients' occupational therapy needs and develop personalized plans of care to ensure optimal rehabilitation and independence.
  • Implement and develop plans of care to ensure quality and continuity of care, and proper discharge planning.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide education and training to patients and caregivers on adaptive equipment, sensory integration, and fine motor coordination skills.
  • Supervise and provide clinical direction to Certified Occupational Therapy Assistants (COTAs), Home Health Aides (HHAs), and clinical staff to ensure high-quality care.

Requirements:

  • Graduate of an accredited Occupational Therapy program.
  • Current state license as an Occupational Therapist.
  • At least 1 year of experience as an Occupational Therapist in a clinical care setting.
  • Home Health experience is preferred.
  • Effective verbal and written communication skills.
  • Ability to travel to meet patient needs within branch coverage area.
