The Role

We're seeking a skilled Nurse Practitioner to join our team, providing primary care to patients in their homes. As a key member of our care community, you'll work closely with our physicians, social workers, and other clinicians to deliver comprehensive care and support.

Responsibilities will include:

  • Providing primary care to patients in their homes, including initial screenings, physical examinations, and ongoing care
  • Collaborating with our care team to develop and implement individualized care plans
  • Ordering and interpreting diagnostic tests, and determining the need for follow-up appointments and further treatment options
  • Prescribing and managing medications, including psychopharmacological agents
  • Updating patient records and ensuring accuracy at each appointment
  • Delivering quality care while maintaining company metrics on documentation, coding, HEDIS measures, and patient engagement
Requirements

We're looking for a highly skilled and compassionate Nurse Practitioner with:

  • Advanced practice experience in geriatric care
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility and adaptability in a fast-paced environment
  • Current licensure and certification in good standing
