Job Title: Home Health Registered Nurse

Fusion Medical Staffing-Home Health is seeking a skilled Home Health Registered Nurse to join their team for a 13-week travel assignment in Delta, CO.

Job Summary:

We are looking for a compassionate and experienced Home Health Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Health Registered Nurse, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care delivery.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care delivery
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Current RN license in good standing
  • At least 6 months of recent Home Health experience
  • Valid driver's license and reliable transportation
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
