Position Overview:
The role involves managing both the Chest Pain Center Program and the Stroke Programs within the healthcare facility.
Key Responsibilities:
As a Cardiovascular Program Coordinator for Chest Pain and Stroke, your duties will include:
1. Acting as the primary coordinator for the designated programs.
2. Engaging with physicians, nursing staff, and the Cardiovascular leadership team to ensure all care aligns with evidence-based practices.
3. Providing and supervising necessary education for patients and their families during hospital stays, and serving as a point of contact post-discharge.
4. Participating in patient care rounds and responding to urgent care activations (e.g., cardiac alerts) to offer support and identify educational needs for staff.5. Collaborating with the director to ensure the program adheres to regulatory standards.
6. Conducting chart reviews and gathering, analyzing, and reporting data for performance improvement initiatives and program evaluation.
7. Working with the director to create and implement evidence-based policies and standards for the program.
8. Assessing, planning, developing, implementing, and evaluating activities that enhance professional development and ongoing competence of care site providers and staff.
9. Maintaining and analyzing data related to the assigned program.10. Serving as the leader and subject matter expert for the program.
Quality Assurance:
Establishing and maintaining a Quality Assurance program and coordinating all certification and accreditation activities for the program.
Minimum Qualifications:
- Associate's degree required; Bachelor's degree in Nursing preferred.
- Current licensure, registry, or certification in the clinical field is mandatory.
- Current Basic Life Support for Healthcare Providers (BLS-HCP) certification from the American Heart Association is required.
- Registered Nurse licensure in the state of practice is preferred.
- ACLS certification is preferred.
- A minimum of 3 years of relevant clinical experience is required, ideally with backgrounds in Cardiovascular, Emergency, or Intensive Care.
Physical Requirements:
Ability to perform tasks including carrying, climbing, crawling, hearing/listening, lifting, manual dexterity, pulling/pushing, seeing, speaking, squatting/kneeling, standing, and walking.
Work Schedule:
Full-time position with a standard weekly schedule.
Compensation:
The hourly rate for this position is competitive and commensurate with experience.
