Job Title: Home Health Physical Therapist

LHC Group is seeking a skilled Home Health Physical Therapist to join our team in Trinity, Florida.

About the Role:

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing and implementing treatment plans, and providing interventions to improve patient outcomes.

Responsibilities:
  • Evaluate patient's functional status, physical therapy needs, and medical conditions
  • Develop and implement treatment plans in accordance with the plan of care
  • Provide services within the scope of practice as defined by state laws governing the practice of physical therapy
  • Observe and report patient's response to treatment and any changes in condition to the nurse supervisor and/or physician
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice and evidence-based techniques
  • Instruct and inform participating members of the healthcare team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals
Requirements:
  • Current Physical Therapy licensure in the state of FL
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
