The Clinical Coordinator plays a vital role in ensuring seamless patient care coordination. Reporting to the Hospital Administrator, this individual will oversee case management activities, facilitate effective communication between healthcare providers and patients, and contribute to optimizing patient outcomes. As part of the interdisciplinary team, the Clinical Coordinator will work closely with nurses, physicians, and other healthcare professionals to identify and address patient needs, develop personalized care plans, and implement strategies to promote optimal health and well-being.

Key Responsibilities
The successful candidate will have the following key responsibilities:
1. Develop and implement effective case management strategies to ensure timely and efficient patient transitions between care settings
2. Facilitate communication and collaboration among healthcare providers, patients, and families to promote informed decision-making and coordinate care
3. Identify and address patient needs through comprehensive assessments, social determinants analysis, and collaborative care planning
4. Utilize clinical expertise and knowledge of evidence-based practices to inform care decisions and optimize patient outcomes
5. Participate in quality improvement initiatives to enhance patient satisfaction, safety, and experience
6. Foster a culture of teamwork and respect among healthcare professionals to ensure smooth operations and excellent patient care
Requirements
To be successful in this role, the ideal candidate will possess the following qualifications:
1. Bachelor's degree in nursing or a related field (Master's degree preferred)
2. Minimum 3 years of clinical experience in case management or a related field
3. Excellent verbal and written communication skills, with ability to interact effectively with diverse stakeholders
4. Strong problem-solving, analytical, and decision-making skills, with ability to navigate complex healthcare systems
5. Experience with electronic health records, computer software, and other relevant technologies
6. Certification in Case Management (CCM) or Accredited Case Manager (ACM) preferred
7. Valid, unrestricted State of Michigan license as a Registered Nurse (RN) or Licensed Social Worker (LMSW)
