At CenterWell Home Health, we're dedicated to delivering personalized care to patients in the comfort of their own homes. As a Home Health Speech Language Pathologist, you'll play a vital role in helping patients recover from illness, injury, or surgery, and regain their independence.

Key Responsibilities:
  • Evaluate and develop individualized plans of care for patients with speech, language, and swallowing disorders.
  • Conduct thorough assessments and provide evidence-based interventions to improve patient outcomes.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Provide education and training to patients and their families on effective communication strategies and techniques.
  • Maintain accurate and timely documentation of patient progress and outcomes.
Requirements:
  • Master's degree in Speech-Language Pathology or related field.
  • Current certification in Speech-Language Pathology (CCC-SLP) or Audiology (CCC-A) from ASHA.
  • Minimum of six months experience as a speech therapist or speech language pathologist.
  • Home health experience preferred.
  • Current and unrestricted license to practice in the state of employment.
  • Current CPR certification.
  • Excellent communication and organizational skills.
