DESCRIZIONE DEL RUOLO

In qualità di Specialista in Coordinamento Intensivo per la Cura, valuti le segnalazioni e contatti prontamente le famiglie. Effettui valutazioni per comprendere i loro punti di forza e le loro necessità, utilizzando IM+CANS per creare e aggiornare piani di servizio personalizzati. Sviluppi e mantieni piani di prevenzione delle crisi e di sicurezza, aiuti le famiglie a identificare reti di supporto e faciliti incontri tra il team di bambini e famiglie ogni 60 giorni o dopo una crisi. Interagendo frequentemente con le famiglie, puoi collegarle ai servizi raccomandati e collaborare con i gestori di casi MCO e altri fornitori. Gestendo fino a 25 famiglie, mantieni registri dettagliati dei casi e fornisci assistenza basata sui punti di forza, culturalmente competente e informata sul trauma, creando un impatto significativo ogni giorno.

  • Laurea in counseling e orientamento, riabilitazione, lavoro sociale, educazione, psicologia o campo dei servizi umani correlato, insieme a due anni di esperienza supervisionata
  • Capacità e volontà di partecipare attivamente in un ambiente di team multidisciplinare ad alta intensità
  • Conoscenza dei programmi comunitari, dei servizi, delle pratiche basate su prove e di una varietà di modalità di trattamento
  • Possesso e mantenimento di una patente di guida valida e fornitura di prova di assicurazione
  • Forti capacità di gestione del tempo
  • Eccellenti capacità interpersonali e di comunicazione scritta
  • Deve possedere le conoscenze, le qualità personali e le competenze pratiche per lavorare efficacemente con professionisti di altre organizzazioni e discipline
