About the Role

We are seeking a skilled and compassionate Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will play a vital role in providing personalized care to patients in their homes, helping them maintain their independence and quality of life.

Key Responsibilities
  • Provide direct patient care under the direction of a Registered Nurse and according to the Aide Plan of Care (POC)
  • Assist patients with daily living activities, such as bathing, grooming, and skin care
  • Communicate effectively with patients, caregivers, and healthcare professionals to ensure seamless care
  • Document interactions, care rendered, and patient progress accurately and thoroughly
  • Practice infection control principles and maintain a clean and safe environment
Requirements
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
  • Good standing on the HHA Registry (if applicable) and completion of HHA/CNA course
  • At least one year of experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency
