Job Summary

We are seeking a full-time day Skill Builder I for our Connxxions program located in Howard County, IN. This position will provide skills and development training to assist patients and/or families in developing skills directed towards achieving developmental milestones and reducing psychosocial barriers.

Key Responsibilities
  • Assist the Therapist in developing, implementing, and revising individual treatment plans, ensuring that services provided are specified in the treatment plan and monitoring progress toward treatment goals.
  • Provide individual and group skills training and development to assist patients and/or families in developing skills directed towards reducing psychosocial barriers, including self-care, daily life management, interpersonal communication, and problem-solving skills.
  • Work collaboratively with the Therapist and medical staff to ensure coordination of integrated care for the patient, including identification of health issues, compliance with treatment, and effective patient-to-medical-provider communication.
  • Complete CANS/ANSA re-assessments and provide consultation to staff on specific cases involving coordination of care.
  • Maintain positive working relationships with staff of other agencies and institutions, homes, and facilities, and act as a liaison between patients and agencies and/or institutions.
  • Prepare and maintain clinical records, reports, and forms, and other administrative tasks as required.
Requirements
  • High school diploma or its equivalent with 2 years paid behavioral healthcare experience or equivalent college coursework toward a human services or health-related degree required at minimum.
  • Associate or Bachelor's degree preferred.
  • A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
  • A commitment to providing individual attention to help clients achieve mental, emotional, and physical well-being.
