Job Summary

Evaluate and treat patients under the order of a referring physician. Develop evaluation and treatment programs using various therapeutic procedures and modalities to restore muscle strength, range of motion, coordination, and body alignment, control pain, and improve movement and function.

Key Responsibilities
  1. Review patient information and evaluate its impact on the assessment and treatment process.
  2. Conduct and document a complete functional assessment of the patient in accordance with established guidelines and protocols.
  3. Design and implement an individualized plan of care containing functional and measurable goals with input from the patient and family.
  4. Perform therapy treatment as ordered by the physician in a timely manner, in accordance with established hospital guidelines.
  5. Meet established key management indicators for productivity levels of 9-patient visits per day.
  6. Supervise, instruct, and train Rehabilitation Technicians in treatment procedures.
  7. Communicate with physicians, patients, families, and other hospital personnel regarding patient progress, problems, and discharge plans.
  8. Follow approved documentation guidelines for the plan of care, progress notes, and discharge summaries.
  9. Orient patients to treatment procedures appropriate to their age and needs.
  10. Inform patients of potential risks during procedures.
  11. Establish and instruct patients, families, and caregivers in home programs and discharge programs.
  12. Assist in the continuum of care process when a patient requires additional rehabilitation following discharge.
  13. Attend and participate in required department meetings and hospital committee meetings as assigned.
  14. Participate in department quality improvement processes.
  15. Provide clinical supervision to Physical Therapy Assistants.
  16. Access continuing education opportunities appropriate to discipline and responsibilities.
  17. Act in compliance with regulatory, corporate, and professional standards and practice guidelines.
  18. Provide input to the Director of Physical Therapy on clinical issues and clinical performance.
