Job Summary:

Corus Health is seeking a highly skilled and compassionate Nurse Practitioner - Patient Care Specialist to join our team. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care, collaborating with our team of nurses and physicians, and ensuring seamless transitions for our patients across various settings.

Key Responsibilities:

  • Conduct thorough patient assessments, screenings, and physical examinations to identify and address patient needs.
  • Develop and implement personalized care plans, taking into account patients' clinical status, personal goals, and preferences.
  • Collaborate with our interdisciplinary team to ensure comprehensive care and effective communication.
  • Perform hospice face-to-face assessments to determine patient eligibility for services.
  • Provide ongoing care information, counseling, and guidance to patients and their families.
  • Assist patients and families with logistical arrangements and coordinate care transitions.

Requirements:

  • Master's Degree in Nursing
  • 2+ years of applicable nursing experience, with at least 1 year of home health experience preferred
  • Valid Nurse Practitioner licensure in the state of New Mexico
  • Driver's License and reliable, insured transportation
  • Strong written and interpersonal communication skills
