We are searching for a seasoned Healthcare Transition Coordinator to join our team of passionate healthcare advocates. In this role, you will serve as a liaison between hospitals and our facilities, ensuring a smooth transition of care for patients requiring extended acute care.

As a key member of our clinical team, you will collaborate with healthcare professionals to develop effective plans of care, focusing on mobility, weight-bearing, ambulation, and nutrition. This expertise enables patients to regain their baseline of health.

Duties and Responsibilities:
  • Evaluate potential patients' medical needs and work with clinical teams to develop effective plans of care
  • Travel to local referring hospitals to assist with discharge planning and identify patients who require extended acute care
  • Collaborate with healthcare professionals to promote patient-centered care and ensure seamless transitions between levels of care
Qualifications:
  • Registered Nurse (RN) or Respiratory Therapist (RRT) License
  • Other educational and clinical designations may be considered with approval of Vice President, Business & Market Development or designee
