We are seeking a skilled and compassionate LPN to join our team at Exeter Hospital. As a Home Care LPN, you will provide high-quality nursing care to patients in their homes, working under the supervision of a registered nurse.
Key Responsibilities- Provide Direct Nursing Care: Supplement and provide direct nursing care to patients, assisting in the nursing process under the supervision of a registered nurse.
- Ensure Timely and Accurate Orders: Ensure patients have timely and accurate orders prior to delivering services.
- Provide Interventions and Education: Provide interventions and education, in accordance with the nursing plan of care, to meet patient outcomes.
- Monitor and Evaluate Patient Condition: Monitor and evaluate the medical condition of patients, documenting and reporting changes to appropriate personnel.
- Collaborate with Healthcare Team: Collaborate with other healthcare team members and registered nurses to adjust the plan of care as necessary to ensure optimal outcomes.
- Assess and Document Patient Needs: Assess patient, caregiver, and/or representative needs and document and report changes to appropriate personnel.
- Meet Agency Obligations: Meet agency obligations for holiday and weekend coverage, complying with agency standards for productivity, patient and staff schedules, and attendance at meetings.
- Enhance Professional Growth: Enhance professional growth and development through participation in educational programs, current literature, community-based and home health agency in-services, meetings, workshops, and providing training to staff, agency, and community groups.
- Participate in Quality Improvement: Participate in the agency's quality assessment process improvement (QAPI) program to improve health outcomes, patient safety, and quality of care.
- 1 to 3 years of direct experience
- LPN License - NH state or other compact state
- CPR certified or within 30 days of hire
- Driver's license - valid
