About the Role

We're seeking a skilled and compassionate LPN to join our team of healthcare professionals. As a Home Health LPN, you'll provide high-quality patient care, working closely with our interdisciplinary team to develop and implement personalized care plans.

Responsibilities
  • Provide home-based care to patients with a range of medical conditions, including illnesses, injuries, and chronic diseases.
  • Work collaboratively with our team of physicians, RNs, CNAs, and aides to deliver comprehensive patient care.
  • Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor.
  • Assist the RN in carrying out patient care plans and teaching patients self-care techniques.
  • Educate family members on patient care plans, medication, and home safety.
  • Collaborate with the interdisciplinary team to coordinate patient care and plan for discharge.
Requirements
  • Graduate of an accredited practical nursing program and active LPN license in Montana.
  • 1 year of LPN experience, preferably in home healthcare.
  • CPR certification.
  • Knowledge of state and federal home health regulations.
  • Clinical proficiency in nursing, strong communication skills, and compassion.
