Job Summary

The Resident Service Coordinator (RSC) is a key member of our team, responsible for coordinating services and supports to residents at assigned housing sites. This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with residents, housing authorities, and community organizations.

Key Responsibilities

  • Service Coordination: Regularly educate residents on the components of the Supportive Housing Program, including emergency response, congregate meals, and resource connections.
  • Facilitation and Coordination: Coordinate and facilitate services and activities, maintaining scheduled office hours and conducting surveys to solicit resident feedback.
  • Resource Referral: Inform residents about State Home Care and other publicly funded programs, making referrals and assisting with benefit securing as needed.
  • Eligibility Screening: Assist in screening residents for Home Care Program eligibility and establish appropriate care plans with AgeSpan Care Manager(s) and Nurse(s).
  • Volunteer Management: Recruit, train, and supervise volunteers who assist with group/congregate activities.
  • Needs Assessment: Identify gaps in services for residents, developing and implementing a range of services and programs to meet unmet needs.
  • Field Visits: Conduct field visits to assess an older adult's physical, social, emotional, and environmental status, determining eligibility and needs in accordance with Executive Office of Elder Affairs regulations.
  • Community Partnerships: Work closely with community organizations and social service providers to provide programs, supports, and services to allow residents to age in place.
  • Documentation and Reporting: Provide accurate and timely documentation to meet all contractual and administrative standards, as well as regular updates, feedback, and reports to housing authorities or property management staff.

Requirements

  • Education: BA/BS Degree preferred; LSW preferred.
  • Language: Bilingual English/Spanish a plus.
  • Experience: Knowledge of and experience with housing/social services with elderly/disabled population strongly preferred.
  • Skills: Solid written, verbal, interpersonal, and team skills; strong computer skills and comfort with Microsoft applications and programs.
  • Certifications: MARSCH (Massachusetts Association of Resident Service Coordinators) Certification helpful but not required; SHINE (Serving the Health Insurance Needs of Everyone) certification a plus.
  • Physical Demands: Ability to lift 25 pounds; access to reliable transportation.
