Our company, Gentiva, is a leading provider of home health services. We are seeking a skilled Physical Therapist to join our team in Lucasville, Ohio. The ideal candidate will have a Master's degree from an accredited Physical Therapy Program and be licensed to practice in the state of Ohio. The Physical Therapist will be responsible for developing and implementing a conditioning/rehabilitation program for patients suffering from various injuries, illnesses, and functional disabilities. The program will be consistent with the physicians' Plan of Treatment and the overall goals of the patient/rehab team. The Physical Therapist will also confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment. **About the Job**

* Develop and implement a conditioning/rehabilitation program for patients suffering from various injuries, illnesses, and functional disabilities.
* Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
* Provide skilled physical therapy services to patients in their homes.

**Requirements**

* Master's degree from an accredited Physical Therapy Program.
* Licensed to practice in the state of Ohio.
* Current CPR Certification.
* One year's experience in hospital or home health is preferred.
