Job SummaryWe are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Clinical Health Specialist. In this role, you will conduct comprehensive health assessments, develop care plans, and provide education to patients and their families. You will work collaboratively with our multidisciplinary team to ensure seamless care coordination and optimal patient outcomes.
Key Responsibilities- Conduct adult and geriatric assessments, medical history, diagnosis, and treatment in patients' homes, skilled facilities, and through telehealth visits.
- Develop and implement individualized care plans, taking into account patients' medical, social, and environmental needs.
- Provide education to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
- Collaborate with primary care physicians, case managers, social workers, and other healthcare professionals to ensure comprehensive care coordination.
- Document patient interactions, assessments, and care plans in accordance with Matrix Medical Network's guidelines and protocols.
- Master's degree in Nursing or related field.
- Current RN and NP licensure in the state of practice, with prescription authority or ability to obtain.
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with electronic health record software.
- Excellent verbal and written communication skills.
