Job Overview

We are seeking a highly skilled and experienced Nursing Director to join our team at Elk Ridge Health & Rehabilitation Center. The ideal candidate will possess strong leadership skills, excellent communication abilities, and a deep understanding of nursing practices.

About the Role

The Director of MDS is responsible for conducting and coordinating the development, completion, and transmission of resident assessments in accordance with state requirements and facility policies. This role requires a strong knowledge of nursing regulations, excellent analytical skills, and the ability to work effectively with cross-functional teams.

Key Responsibilities

• Develop and implement resident assessment plans that meet or exceed quality standards
• Conduct thorough assessments of residents' medical, nursing, mental, and psychosocial needs
• Collaborate with healthcare professionals to develop and update care plans
• Evaluate resident condition and adjust assessment plans as needed
• Serve as a liaison between departments to ensure seamless communication and coordination

Requirements

• Bachelor's degree in Nursing from an accredited institution
• Minimum 2-4 years of experience in Skilled Nursing environment, preferably with supervisory experience
• Current, unencumbered LPN license in Colorado; RN license preferred
• Valid CPR certification
