Homecare Nurse

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Care RN. As a Home Health Care RN, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Key Responsibilities:

• Conduct thorough assessments of patients' physical and emotional needs
• Develop and implement personalized care plans to meet patients' unique needs
• Provide skilled nursing care, including wound care, medication management, and education
• Collaborate with patients, families, and healthcare teams to ensure seamless care transitions
• Document patient progress and outcomes in a timely and accurate manner

Requirements:

• Current RN license in good standing
• Experience in home health care or a related field (not required)
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Flexibility to work varied schedules, including evenings and weekends
