Job Title: MDS Coordinator

Job Summary:

Devonshire Care Center is seeking a skilled MDS Coordinator to participate in the Resident Assessment Process. The successful candidate will be responsible for completing the Minimum Data Set (MDS) and accompanying Care Area Assessments (CAAs), as well as directing the Resident Assessment Protocol Summaries (RAPS).

Responsibilities:

  • Complete MDS items, RAPS, and Care Plans as designated by HHC.
  • Participate in resident Care Plan Meetings.
  • Train new members of the IDT on clinical content and electronic completion of MDS items, RAPS, and Care Plans.
  • Print Nursing Home Quality Indicator Reports from the State database and distribute to the facility Director of Nursing and Administrator.
  • Maintain resident confidentiality and privacy.
  • Comply with applicable legal requirements, standards, policies, and procedures.

Requirements:

  • Ability to communicate with residents and personnel.
  • Ability to remain calm under stress.
  • Regular attendance as scheduled.
