About the Role:The Patient Care Manager, RN will be responsible for the day-to-day operations of our hospice site, ensuring the provision of quality care to patients and their families. This role requires strong clinical and leadership skills, as well as the ability to manage and oversee clinical and administrative functions.
Key Responsibilities:- Manage the site in the absence of the Executive Director, Administrator, or Senior/Executive Patient Care Manager.
- Oversee clinical and administrative functions, including recordkeeping and quality assurance.
- Respond to phone referrals and inquiries in a timely and professional manner.
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
- Registered Nurse with a minimum of Associate's Degree in Nursing required from an accredited school of nursing.
- Two or more years' experience in hospice required.
- Three years' experience in direct patient care in a clinical setting required.
- Current license to practice nursing in the state of Texas.
