The Home Care Licensed Practical Nurse at Emerson Hospital provides direct nursing services to individuals and families in the home and in the community. This position reports to the Nursing Supervisor, Home Care, in conjunction with the patient's primary nurse.
Key Responsibilities
- Deliver high-quality patient care in a home setting, adhering to established standards and protocols.
- Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to address these needs.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Provide education and support to patients and their families, promoting health literacy and self-management skills.
- Maintain accurate and detailed records of patient interactions, treatments, and outcomes.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
Requirements
- Education
- Graduate of an approved practical/vocational nursing program.
- Experience
- Minimum two years of medical/surgical experience required.
- Knowledge and experience in the practice of community health nursing preferred.
- Home care experience preferred.
- Licensure and/or Certification
- Current MA license to practice as practical/vocational Nurse.
- BLS healthcare provider required.
