Job Summary

We are seeking a skilled Occupational Therapist to join our team at Cornerstone Rehab. As a key member of our clinical staff, you will be responsible for delivering high-quality care to our patients and collaborating with a diverse team of professionals.

Responsibilities:

  • Provide occupational therapy services to adult and geriatric patients in skilled nursing facilities
  • Develop and implement individualized plans of care to meet patients' needs and goals
  • Document patient progress and maintain accurate records
  • Collaborate with other healthcare professionals to ensure comprehensive care

Requirements:

  • Valid State Occupational Therapist License and Credentials
  • Experience in skilled nursing and/or acute care settings
  • Covid Vaccine required in certain locations
