Senior Nurse Practitioner

We are seeking a compassionate and skilled Nurse Practitioner to join our team at Abode Care Partners.

As a Nurse Practitioner, you will play a vital role in providing high-quality medical care to seniors and individuals with complex needs.

• Plan, direct, and evaluate patient medical needs to ensure optimal health outcomes.
• Treat chronic and urgent episodic illnesses, injuries, and accidents with compassion and expertise.
• Order and interpret laboratory and diagnostic testing to inform patient care decisions.
• Collaborate with caregivers and other healthcare professionals to provide seamless patient care.
• Ensure patient medical records are accurate and up-to-date.

• Active state Nurse Practitioners license.
• Master's degree in Nursing (MSN).
• Five or more years of nursing experience.
• Strong interpersonal communication skills.
• Proficiency in medical instruments and equipment.
