- Provide outpatient services exclusively to the geriatric population.
- Encourage proactive patient engagement with a minimum of four wellness visits annually, immediate access for urgent care, and comprehensive transitional care management.
- Maintain a manageable patient panel size, allowing for enhanced patient interaction and thorough evaluations.
- Typically, physicians will consult with 12 to 15 patients daily, with a capped panel of around 600 patients.
Our approach ensures that a smaller patient panel translates to more time for each patient, leading to more accurate diagnoses and a stronger emphasis on preventive care.
Position Details- This is a FULL-TIME permanent opportunity.
- Standard working hours are Monday to Friday, from 8:00 AM to 5:00 PM.
- No weekend or evening shifts required.
- No hospital duties are involved.
