Gentiva Hospice is dedicated to providing exceptional care to patients and their families. As a Per Diem Hospice LPN, you will play a vital role in delivering compassionate and personalized care to those in need.

About the Role

This is a flexible, per diem position that allows you to choose your own schedule and work at your own pace. As a Hospice LPN, you will provide direct patient care services, evaluate patient needs, and assist in the preparation and execution of the plan of care. You will also report findings or changes in the patient's condition to the Registered Nurse, interdisciplinary team, and referring physician, and document all reports.

Requirements

To be successful in this role, you will need to be a compassionate and empathetic individual with a strong commitment to patient-centered care. You will also need to be comfortable providing care in a variety of settings, including patients' homes. A minimum of one year of patient care experience in hospice, home health, ICU, geriatric, or oncology is preferred, as is a current driver's license and automobile insurance. Additionally, you will need to be licensed to practice as an LPN in the state where you will be working.
