Medical Director, Post-Acute Care Specialist

Theoria Medical seeks a highly skilled Physician to serve as a medical leader in our post-acute care services. As a key member of our team, you will oversee the delivery of high-quality primary care services to residents in skilled nursing facilities across the country.

- You will be responsible for providing comprehensive primary care services, collaborating with nurse practitioners and physician assistants to manage resident care, and supervising the work of other healthcare professionals.
- A competitive compensation package, ranging from $50,000 to $400,000 per year, depending on employment status and availability.
As a Physician with Theoria Medical, your primary responsibilities will include:
- Providing high-quality, patient-centered primary care services to residents in skilled nursing facilities.
- Collaborating with nurse practitioners and physician assistants to develop and implement effective care plans.
- Supervising the work of other healthcare professionals, including nurse practitioners and physician assistants.
- Participating in quality improvement initiatives and programs to ensure exceptional patient outcomes.
- Degree in Medicine (MD) or Doctor of Osteopathic Medicine (DO).
- Licensure to practice medicine in the United States.
- Board certification in Family Medicine or Internal Medicine.
- At least 2 years of experience in primary care, preferably in a post-acute care setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
