Inpatient Case Management Nurse Specialist

Job Summary:
We are seeking an Inpatient Case Management Nurse Specialist to join our team at MEDSTAR HEALTH. The successful candidate will work collaboratively with healthcare professionals to develop and implement care plans that promote quality, cost-effective patient outcomes.
Responsibilities
Serves as a member of the multidisciplinary Case Management Team and applies clinical expertise and medical appropriateness to care coordination and discharge planning.
Facilitates the delivery of quality, cost-effective, patient-centered care from pre-admission through post-discharge timeframe.
Ensures care is designed to meet individualized patient outcomes by monitoring care and services delivered to selected patient populations during the acute hospital stay.
Promotes effective case management and utilization of resources, and works to achieve optimal clinical and resource outcomes for the acute, and post-hospital phases of care.
Collaborates with healthcare professionals to develop and coordinate care plans that address patient needs across the continuum.
Evaluates and documents patient response to care plan and achievement of outcomes, making recommendations for modifications as indicated.
Maintains knowledge of regulatory agencies' requirements, necessary criteria for admission to various care settings, and reimbursement methods for different levels of care.
Manages own professional growth in managed care, care management, healthcare financial trends, clinical practice, and research.
Participates in performance and service improvement teams, assisting in program evaluation through customer service surveys, data analysis, and best practice/benchmark data.
Requirements
Associate's degree in Nursing (ADN) required; Bachelor's degree in Nursing (BSN) preferred.
Ability to use computer to enter and retrieve data.
Working knowledge of Microsoft Word, Excel, and PowerPoint applications.
Contributes to achievement of established department goals and objectives, adhering to department policies, procedures, quality standards, and safety standards.
What We Offer
Competitive salary range: $83,000 - $110,000 per annum, based on experience.
