Job Summary

Patriot At Home is seeking a skilled Registered Nurse to join our team as a Hospice Case Manager. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in need of hospice services.

Responsibilities
  • Coordinate and plan hospice care for patients, working closely with the healthcare team and family members.
  • Evaluate the effectiveness of hospice services and make recommendations for improvement.
  • Conduct thorough assessments and evaluations to determine patient needs and develop individualized care plans.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in IDG and case conferences to discuss patient progress and develop care plans.
Requirements
  • Graduate of an accredited School of Nursing with a current RN license in the state.
  • One or more years of experience in hospice care, preferably with a focus on case management.
  • Knowledge of Medicare and Medicaid guidelines, as well as state-specific regulations.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Valid driver's license and reliable transportation.
