ED Navigator LPN

We are seeking a dedicated and compassionate ED Navigator LPN to join our team at Ochsner Health. In this role, you will play a vital part in streamlining patient care transitions and redirecting patients to appropriate levels of care.
As an ED Navigator LPN, you will be responsible for conducting initial evaluations, engaging patients and families, and identifying patient needs. You will also connect patients to appropriate resources and utilize behavior change strategies to educate patients and families.

Key Responsibilities:
- Conduct initial evaluations and engage patients and families

- Adapt behavior to specific patient populations
Requirements:
- Associate's Degree in Nursing or related field
- 1 year of experience in a clinical or social services setting
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
